The NBA strives to provide a quality basketball program that is enjoyable for all players, spectators, coaches, and officials alike. Use this form to report any complaints you have about the league or incidents that need to be investigated by the league coordinators. Please be as objective and factual as possible.

Please be as descriptive as you can concerning your complaint. If this is an incident involving a referee, give the referees name if known, the nature of the call or rule involved and any other information you feel we need to know. If the incident involves a game, please give the scheduled time of the game, opposing team # and score of the game.
